on Myspace I'm trying to find a survey type generator.?

I was thinking of something like a survey generator. I am a DJ and want to know ppls fav artists and songs when they visit myspace. be able to have them type in answer then submit and have it emailed to me...any suggestions?

Well, just make a seperate email for people to send ideas to.
